The referral form needs to be completed by either:
Medical and allied health clinicians may need to contact or refer your patients to a local community dental clinic in the first instance for a full dental examination and to obtain relevant diagnostic information to support your referral.
When we receive your referral, we will review it to ensure we have all the information needed as a prerequisite.
It may take up to six weeks to process a referral.
All prerequisite clinical and diagnostic information must be provided as part of the referral. Referrals may be delayed or returned if supporting information is not available. The referral will be returned to the referral clinic with a request for more information.
Each Specialist unit, disciplines and the dental teaching clinic will have different prerequisites and clinical criteria. For more detailed information, please visit our Specialist dental service page.
When we accept your referral, you will receive a confirmation letter. We will add you to the appropriate waiting list.
You may be contacted to attend a screening appointment for a clinical or exam. This is not an acceptance for specialist treatment by RDHM.
